For quality improvement, compliance, credentialing, and administrative operations.

Public health reporting, court orders, law enforcement, etc.

3. Uses Requiring Authorization

We will obtain your written authorization for:
- Marketing unrelated to your care.
- Release of psychotherapy notes (if applicable).
- Any uses not described in this notice.

You may revoke authorization at any time in writing.

4. Your Rights

You have the right to:
- Access and obtain copies of your medical records.
- Request corrections.
- Request confidential communications.
- Request restrictions (not all requests must be granted).
- Receive an accounting of disclosures.
- File a complaint.
